This form can be used following an event to evaluate how succesful the event was. If kept in file, this form can be extremly helpful in planning the same event the following year.

In children's ministry we plan many events. Camps, crusades, VBS, outreaches, the list goes on. This form was created to assist in evaluating the success of the events we work so hard to put on for our churches.

If this form was completely filled out and filed away for the following year, it could be a great tool in planning the same/similar event.
